For some, that may mean working toward a full-time education setting; for others, it may involve a more gradual or alternative route. We monitor each learner’s journey closely, adapting their plan as needed to ensure consistent support, growth, and development.

Step 1: Initial Information Gathering & Engagement

Build a clear understanding of the learner’s starting point, strengths, challenges and aspirations before teaching begins.

Key Process:

  • Review existing information (e.g., EHCPs, reports, previous provision notes)
  • Engage with parents/carers, professionals, and the learner where appropriate
  • Establish initial goals and key areas of focus
  • Prepare for a positive and well-informed introduction to learning

Step 2: Individual Learning Plan Development

Create a flexible and personalised plan that supports the learner’s strengths, interests, and needs.

Key Process:

  • Tailor a curriculum framework that matches the learner’s ability, interests, and readiness
  • Identify suitable subject areas and delivery methods
  • Set clear, short-term goals that support engagement and progress
  • Agree on long-term outcomes such as qualifications, skills development, transition goals, or personal growth targets
  • Define how progress will be reviewed, recorded, and shared

Step 3: Tailored Learning Experience

Begin tuition with a focus on relationship-building, engagement, and identifying starting points for learning.

Key Process:

  • Start tuition at a pace that suits the learner’s readiness
  • Carry out informal and formal baseline assessments once the learner is comfortable and engaged
  • Adapt content and teaching methods based on assessments, ongoing observations and learner feedback
  • Offer flexible delivery (e.g., online, in-home, or community spaces) depending on learner needs

Step 4: Personal Growth & Wellbeing

Promote holistic development alongside academic progress.

Key Process:

  • Build trust and positive relationships between tutor and learner
  • Support development of self-esteem, independence, and education resilience
  • Encourage learners to reflect on their progress and celebrate achievements
  • Ensure emotional and social wellbeing is central to the learning experience

Step 5: Progress Monitoring & Transition Planning

Identify and support each learner’s next steps, based on their progress and goals.

Key Process:

  • Regularly review progress and adjust learning plans
  • Involve learners and families in setting new targets
  • Explore next-step options such as progression to full-time education, vocational routes, or continued provision
  • Ensure transitions are well supported and learner-led

Step 6: Integration into Educational or Vocational Settings

Support learners toward a positive and appropriate next phase of education or training.

For some, this may be full-time education in another setting. For others, it may involve continuing with AP Education while building readiness over time. Our role is to ensure each transition is purposeful, supported, and based on the learner’s best interests.

  • Provide regular progress check-ins and adapt goals as needed
  • Support gradual transitions or continuing provision based on learner needs
